The competent authority expects major accident hazard sites to take all measures necessary to prevent and mitigate the consequences of major accidents. This includes Human Factors, and applies to warehousing, distribution, blending, etc, at upper and lower tier COMAH sites.
Human Reliability Analysis (HRA) is a qualitative approach used to identify potential human failures that may occur when a task is undertaken, identify and categorize the types of human failure, the consequences of those failures, and develop actions to reduce the likelihood and/ or mitigate the consequences of a failure.
